All natural persons in the United States and United States territories who are or were current and former residents of the McFarland Female Community Re-Entry Facility whose personal and protected health information – including first and last names, dates of birth, symptoms, medical or mental diagnoses, treatments and conditions – was misplaced in or around the week of May 9, 2016 (“Data Breach”). YOUR LEGAL RIGHTS AND OPTIONS IN THIS SETTLEMENT
|Participate in the Settlement||If you wish to receive any of the benefits being offered in connection with the settlement, read this website for information on how to sign up for credit protection and/or submit a Claim Form for reimbursement of expenses.|
|Exclude Yourself from the Settlement||If you do not want to participate in the settlement, you must send a letter requesting exclusion postmarked no later than September 27, 2018 or else you will be bound by the settlement.|
|Object to the Settlement||If you wish to object to the settlement, you must follow the directions in the notice. The deadline to object is September 27, 2018.|
|Participate in the Final Fairness Hearing||If you submit a timely objection to the settlement, you may also indicate in the objection whether you wish to appear in court and be heard at the time of the final fairness hearing.|
|Do Nothing||If you do nothing with respect to the Notice, you will not receive any settlement benefits and you will be bound by the terms of the settlement including the release of claims.|
